VOUCHER CLIENT PARTNERSHIPS

Thank you for wanting to partner with us to provide free/low-cost spay, neuter, and vaccines to low-income residents and their pets! Please complete this form in order to be contacted to set up a voucher partnership.

CLINIC LOCATIONS

1. South King County (Renton) 3^rd Sun/month Mostly cats with a few spots for small dogs under 20lbs

2. Pierce County (Puyallup, Spanaway, Roy, Bonney Lake, Lacey, Lakewood, Lakebay) Monday-Thursday

3. Snohomish County (Everett, Sultan, Tulalip, Marysville) Tuesdays

4. Nomadic Neuters (Thurston, Mason, Jefferson, Lewis, Grays Harbor) 1^st Tues/month *Cat Rescues Only

OUR LIMITATIONS

• Dogs must be under 60lbs female or 70lbs male to be seen on the Spay Stations.

• Blood work is required for pets over 6 years old or a waiver must be signed.

BILLING

You will be emailed an invoice monthly by the 15^th. We ask that you pay your balance in full every month. A balance carried for more than 45 days can impact our partnership and we may contact you about pausing until the bill is paid in full.
